  • Explain how principles of support are applied to ensure that service users are cared for in Fleetwood hall home

Principle of support can be defined as norms, rules, and values that   guide staffs in the caring process in health and social care practices ( Cambridge training  and development 2000). There are seven principles of support which professional working within health and social care system must practice. They are   promoting communication and relationship, engaging in anti-discriminatory practice, Maintaining confidentiality of information, protecting individual from harm, provide person-centred care, respecting diversity, and promoting individuals rights to choice, respect, dignity and independence (Kina 1996, Cambridge training and development 2000). Promoting communication and relationship is an essential part of the caring process.   Under this principle, professional working within health and social care must   value the way they communicate to individuals and should also strive to establish a positive relationship with the service users (Cambridge Training and development).  In Fleetwood Hall Home,   workers did not do as required by this principle.   They took a practitioner-centred approach where they made most of the decision without gathering and communicating with service user to understand their point of view.  To improve services, Fleetwood hall home will need to educated it staffs on how to take a consultative and participative approach when communicating where they also listen to residents view before making decision Health or social care services need to be provided in manner that support and promote individuals rights to choice, independence, respect and dignity ( Kina  1996). Again, this principle was not practiced in Fleetwood hall home. For instance, CQC found that residents were not consulted in care planning which mean staffs were not supporting or promoting residents’ rights to choice of service.   For improvement of services, Fleetwood hall home must ensure staffs support and promote the rights of residents.  This can be achieved by training the staffs, supervising them and constantly reminding them of their duty to promote residents rights. Health and social care services has a duty to protect individuals from harm. Under this principle, care services are required to do whatever is necessary including risks assessment to ensure individuals are protected from all form of harm (Kina 1996). Again, in Fleetwood hall home this principle is not practiced properly.  In its report CQC noted that medicine was not administered in safe way. Also, it was also noted that in part of the corridors, floor was moving creating risks of harm to people with limited mobility.  In addition, Fleetwood hall home lacked sufficient number of staffs to manage risks presented by some people.  In order to improve the services, Fleetwood hall home will develop and implement risks management system. This will help in risk identification and elimination.

1.2 Outlines the procedures that are adopted in Fleetwood hall home for protecting clients and colleagues from harm

Professional in health or social care has a duty to protect individuals from harm (Kina 1996).  Harm is a term used to describe any form of physical or mental damage or injury caused to individuals (Petersen and Wilkinson 2007).   In Fleetwood hall home, there are several cases of harms that have occurred. For instance, some residents in the dementia units had unexplained bruises in their body.  In addition, there is also allegation of wrong medication in home where a resident was given medicine that GP has discontinued on her (Care quality commission 2014).    All the above cases are an indication that Fleetwood hall home was not taking necessary step to ensure proper protection of residents against harm. As manager in this institution, there are several guarding intervention s to ensure residents are well protected.    A complain and reporting procedure will be put in place. This will give residents and their families in Fleetwood hall home an opportunity to report cases of abuse by staffs or by others residents to the management.   Also,  regular  training  will be conducted to improves staff skills and knowledge  on   protection of  vulnerable adults and children,  how to professionally handle  challenging behaviour and  also on the use of restrain.

  • Analyse the benefit of following a person-centred approach with the service users of Fleetwood hall home.

Over the recent year, person-centred approach has become the norm of health and social care practice. It is described as a way of thinking and doing things  that perceived  users of health and social care services as  equal partners  in planning, executing , monitoring and evaluating care  to ensure  it  meets their needs ( Health innovation network  2013). This simply means putting individuals at the centre of decision making relating to their care.    When person-centred approach is sued within health or social care, it has several benefits to services users. First, person-centred empower service users. It gives individuals and their carer greater control   over their own live. As a results, individuals are able to make choices of services they need to access and determine how and when they are delivered (Health innovation Network 2013), simply put, person centred approach promote   individuals rights to choice. Secondly, person-centred approach is great way of promoting communication and improving the relationship between service users and service provider.  It create a conducive environment of honesty, transparency and  inclusivity  that allows exchanges of information and formation and  maintenance of positive relation between people using services and those providing services (Brooker 2007). Thirdly, It encourage service users and their families to take a leading role in their life and health since they feel parts of the decision making process. Since person –centred is anchored on  service user participation in decision making, individuals feel  parts of the decision made and  as such  they play an important role it execution (Health innovation network  2013).

  • Explain any ethical dilemmas and conflict that may have arisen when providing care, support and protection to the elderly clients in Fleetwood hall home.

Ethical dilemma is complex situation where one is presented with two options but the correct choice is not immediately obvious (Trish 2010).   This meaning having two options but choosing one option mean transgressing the other option. A case of ethical dilemma exists in our case study.   Under the principle of support, care professional are required to take necessary measures to protect vulnerable patients against harm.  Therefore, when handling and moving frail patients it is essential to use sling to prevent fall accidents.  However, Mrs, Y who is frail refuses the use of the sling on her.  This put care professional in an ethical dilemma since they cannot use the sling against her will.  This is mainly due to principle of support which require care professional to support and promote individuals rights to choices.  In this case an ethical dilemma is apparent whereby the  staffs has to chose between fulfil their duties of protecting  patient form potential harm or promoting patients  rights to choice.

There are a range of legislations, regulation and policies   relevant to health and social care practice. Fleetwood hall home must ensure their implementation.   Below is an explanation of how Fleetwood hall home will implement them.  Implementation of health and social care Act 2008 The health and social care 2008 provide a framework for regulating quality of care services.  One of the areas addressed by this Act is infection control and management. The Act introduced the code of practice on prevention and control of infection.  Under this code of practice, service providers have the obligation to develop systems to prevent and control infection. This system will help in identification and elimination of infection related risks.  Secondly, Fleetwood will have to maintain a clean and appropriate environment that makes it impossible for infection to occurs or spread.  Thirdly, Staffs training on infection control and prevention will be required to ensure all staffs   engage in safe practice.  Fourthly,     it should develop and ensure adherence to policies for infection prevention and control. Under this policy, care organization has the obligation to develop system to manage and monitor the safety of vulnerable Adult under their care.  In Fleetwood hall home, there are several cases where harm has been caused on vulnerable adult. This shows failure by the institution to implement SOVA requirement. To put this Policy into practice in Fleetwood Hall home, an organization policy and procedure for assessing and indentify case of abuse is needed. Furthermore, the care home will have to work closely with police and other relevant agencies to ensure. Furthermore, staffs training of safeguarding vulnerable adults patients will be   required.

It should start by reading and understanding the national policy requirement as contained in the Act. This will   help understand what is expected of them by the Act. Then, an internal audits need to be conducted to identifies residents needs for protection from infection. This audit should aim to identify adults who are susceptible to infection. This would help develop policy that is responsive to residents needs. It is also important to engage the staffs so as to get their views and opinions.  This is essential as it ensure everybody views are heard therefore reducing resistance during policy implementation. Drafting and editing the Policy should then be done. While doing so, it is important that national policy requirement, staffs opinion and idea, and residents’ needs are taken into consideration. The policy or procedure will then be communicated to staffs for implementation.  This should be followed by periodical review of   the policy to ensure it still relevant and effective and where necessary, appropriate revision should be done.

Policy, legislation, regulation and code of practices are meant to ensure service providers   meet the needs of service users.   There are therefore very crucial within health and social care practice. Also, it is through them that  service provider  are left with no option but to provide  service users with  quality  services that is caring, effective, safe and well led.  In simpler words,   legislations, regulation and code of practices relevant to health and social care services has provided statutory guideline that has help service providers improve the quality of their services hence meeting the needs of their clients.  In Fleetwood hall home, legislations, regulation and codes of practices will helps  the care home  and it staffs to  treat  residents with respect and dignity and  meet their  holistic needs. Despite the above positive impacts, Legislations, regulations and code of practices will impact Fleetwood hall home in negative way.   First, to adhere to them, there will be an increase in business costs.  The costs incurred in running Fleetwood hall care will increase drastically as the care home strive to adhere to the legislations and regulations. For instance, CQC regulation requires it to hire additional staffs to improve safety of residents. This will lead to an increase into payroll cost.

Theses theory took psychological perspectives that emphasis the study of the whole person and their uniqueness. In health and social care practices, Humanistic theory emphasis the important of focusing of Individuals needs when planning for care services. In other word, the theory rejected the practices of generalization where services was designed to cater for the mass rather than meet the needs of each individual In addition, the Erikson's stages of psychosocial development theory have also had important impacts in health and social practices.  Psychosocial development theory   explains how identity is formed.  The theory says that there are eight stages of development and in each stage there are challenges and crisis.   Personality or identities are formed as individuals deal with the challenge and crises in each stage. If crisis in particular stages are not well dealt with, they might manifest themselves as   behaviour problem in future.  In health and social care practices, this theory emphasis the important of understanding one’s ecological and culture upbringing to offer services that in caring and responsive to the unique of each person hence meeting the need of the diverse population.

Human life is full of social processes.  Bardis (2014) defined social process as recurrent form of social interaction that occurs again and again in group.   Social process especially those related to poverty, unemployment, lack of education, poor health etc can lead to marginalization, isolation and exclusion within the society (Currer 2007)   Social process do affects service user within heath and social care in several ways. For instance, service user involvement in the care planning and execution has promoted a person-centred approach that empowers patients to decide the care best for them. Additionally, social process can lead to isolation of the minority. However, the service user involvement as practiced within health and social care has helped remove culture imposed barriers that  usually lead to isolation  of some people hence make it impossible top meet the needs of the divers population. Secondly, when people from different discipline work together, they are able improve their skills in giving quality health care to patients and their families.  Also,   as people from different discipline work together,    they are able to have an understanding of what other professional does and as such they develop respect for other professionals (Day 2013). Also, as highlighted by Leathard (1994), staffs who work in inter-professional team feel more satisfied with their job as compared to others. Despite having some advantage, Inters-professional working also has several disadvantages.   One of the biggest disadvantages is conflict between partners.   As people from different discipline work together conflict may arise due supremacy battle between different professions (Pollard et al 2009).  In addition, there might be dissatisfaction with outcomes. For instance, Fleetwood Hall home may experience delay in referring a patients whilst they wait for healthcare facility to respond. Although the inter-professional working has both advantages and disadvantages, the advantage outweigh it disadvantages and as such it remain an important part of service delivery within health and social care. Better results can be achieved through it if disadvantage were minimised by  creating shared goal among partners, and creating awareness  among the different discipline on how their practice affect others.

Where the practices or conduct does not conform to the policy,   we sit down with members to discuss reason for non-conformity and the best ways forward.
